Dr. James I. Meyer:

Dr. Meyer joined PVIM in 1996. Dr. Meyer graduated from the University of Minnesota Medical School in 1986 and completed his residency with Fitzsimmons Army Medical Center in 1989. He is board certified in Pulmonary Medicine, Sleep Medicine and Internal Medicine. Dr. Meyer believes in being open and honest about a patient’s condition and being liberal about ordering tests. On his days off, Dr. Meyer can be found fishing and enjoying time with his family.

Dr. Shad T. Grubbs:

Dr. Grubbs joined PVIM in 2002. Dr. Grubbs graduated from University of Texas Medical School at Houston in 1998 and completed his residency at the University of Colorado Health Science Center in 2001. He is board certified in Internal Medicine and Geriatrics. Dr. Grubbs believes in providing top notch care for the elderly. When not in the office or nursing homes seeing patients, Dr. Grubbs will be trekking through the mountains hunting. Dr. Grubbs also enjoyings spending time with his family and coaching his childrens sports.

Dr. David A. Chavez

Dr. Chavez graduated from University of Colorado School of Medicine in 1997 and completed his residency at St. Anthony Family Medicine in 2000. He is board certified in Family Medicine. Dr. Chavez strives to fulfill his philosophy of empowering his patients to make well-informed decisions about the healthcare choices available to them so they can actively participate in their healthcare. When not taking care of patients, Dr. Chavez enjoys backpacking, hiking, fishing, skiing, reading, and international travelling with his family.

Dr. Chavez speaks Spanish fluently and welcomes Spanish speaking patients.

Andrea Mantick, MSN, FNP-C

Nurse Practitioner

Andrea joined PVIM in 2020. Andrea earned her nursing degree in 2007 from Weber University. In 2018, Andrea earned her Masters of Science in Nursing through Chamberlain University with emphasis on Family Medicine. On Andrea’s days off she enjoys hiking, snowboarding, and sports activities with her family.

Megan Vincent, FNP-C

Megan joined PVIM in 2024. Megan earned her Bachelors of Science in nursing degree in 2019 and her Masters of Science in Family Nursing Practice in 2023, both from the University of Colorado. On Megan’s days off she enjoys time with her daughter and husband, running, gardening, cooking, and baking.
